The administrative complaints are to be heard in a State Fair Hearing.

ACF as a group of concerned parents have seen abuse of the same type occurring to children and therefore we have prepared a general complaint form which outlines these abuses that are harming our children.
Please check the boxes which apply to your specific case in which you have specific proof that these actions are occurring.
There are also blank spaces in which you can write other specific complaints or issues, which apply to you or your child's case.
Make sure that you completely fill out all of the needed information with your name and address then date and sign the complaint.
Make a copy for your self and then mail your complaint to the appropriate agencies. We have provided a list of potential agencies and they are in the order in which you should file complaints.
