Petitioner hereby requests, of the honorable __________________________, Judge/Commissioner of the above-entitled court, discovery of documents within the Social Worker’s file. The documents requested are as follows:

1)  Copies of all 909 forms and of writings of any and every kind, of each and every social worker and CPS worker, regarding the children and/or adults in this case, contained therein, inclusive of the notes and records of social worker ______________________________.

2)  Copies of any correspondence from or to the Children’s therapists (both past and present) contained therein.

3)  Copies of any and all reports and or evaluation contained therein.

4)  Copies of any and all correspondence, reports, or any other writings to or from the present foster parents, including any logs contained therein.

5)  Copies of all records, notes, drawings, raw data, and of writings of any and every kind of __________________, and from any other therapist, psychologist, and evaluator for my children, and myself contained therein.

6)  Copies of any and all supervised visitation forms, correspondence, and or notes contained therein.

7)  Copies of any other writings, correspondence, or things not presently known or in the Juvenile court file, contained therein.

8)  All records used by social workers, therapists, doctors or any other person or persons in preparing reports to the court or to social services.

Petitioner hereby requests the above named documents and things, based on their extreme relevance, coupled with their urgent need to resolve this matter, in fairness to all parties concerned, and most importantly, in the best interest of the children.

Petitioner makes this request pursuant to: § 827(a)(1)(D)&(E); §§ 10850 et seq.; California Rule(s) of Court; Rule 1420 et seq., more precisely (a), (c), (d) (1)-(7), (f)-(k); also see 62 Ops.Cal.Atty.Gen. 634; In re Maria V.
